How do I gain access to my rare cancer patient's CART-Wheel questionnaire?

You need a CART-Wheel.org account to login and view your patient’s questionnaire. You can request an account by emailing contact@cart-wheel.org with your full name and preferred email address. We will set up an account for you and email you with instructions on how to activate it and login. From there you are able to register patients and enter information in their CART-Wheel questionnaire.

Alternatively, you may be nominated by a patient completing a CART-Wheel.org Consent Form and be contacted directly by us with an invitation to activate an account with us.

Patients can give a named clinician permission to view, enter and edit their data by ticking 'yes' at consent option 2 on the Consent Form. Once you have activated your CART-Wheel.org account and login, you will see your patient’s name and can view and edit their questionnaire. When additional patients of yours register with CART-Wheel, you can use the same account to view and enter information into their questionnaire.

Can I use my patient’s CART-Wheel information for research purposes?

You can access de-identified data for research purposes via the standard BioGrid Australia project access processes.

After I register my patient, how do they access their questionnaire?

When you register your patient, you create a CART-Wheel.org account for them. If your patient wishes to access their account either to enter information or view what you enter, they will need the username you gave them and a password. A password can be requested from the CART-Wheel.org office by emailing contact@cart-wheel.org with the patient’s full name. Either patient or clinician can send an email with this request. The password will be emailed to the patient.

*Please note that if a patient requests a password, they will need a registered email address.

Does my patient need to enter information in the questionnaire?

No. It’s great if they are interested and would like to do this, but it’s not essential if you are able to enter information on their behalf as their approved clinician. Patients can nominate a clinician to view, enter and edit their information by ticking consent option 2 on the CART-Wheel Consent Form. Patient information cannot be used for research purposes until a Consent Form has been received.

All information entered into the CART-Wheel.org questionnaire is valuable to rare cancer researchers, whether entered by a patient, their clinician or a combination of both parties.

What if my patient is not interested in entering their medical information into the CART-Wheel questionnaire?

Some patients do not wish to enter their information into the CART-Wheel.org questionnaire. They may be happy for someone to do this on their behalf. Patients can nominate a clinician to view, enter and edit their information by ticking consent option 2 on the CART-Wheel Consent Form. Patient information cannot be used for research purposes until a Consent Form has been received.

All information entered into the CART-Wheel.org questionnaire is valuable to rare cancer researchers, whether entered by the patient, their clinician or a combination of both parties.

How long will it take to complete the questionnaire?

The questionnaire includes 21 questions (sections). Not all of these questions may apply to your patient.

Depending on how much information you can provide, it will take about 30 to 60 minutes. You do not have to answer all questions in one session. You can return to the CART-Wheel.org website, login and view your patient’s questionnaire at any time. You can continue the questionnaire or edit information already entered.

I have limited time to enter information. What are the most important questions to complete?

All the questions ask for information which will be useful for rare cancer researchers and we greatly appreciate your time entering this data. At a minimum we would like you to complete the questions on diagnosis (question 3) and tumor location (question 4). Treatment details (question 9) would also be a priority to complete. If your time is limited, we ask you to consider returning to the questionnaire at another time or nominating a member of your treatment team to enter the patient’s information on your behalf.

If you would like any members of your treatment team to be able to enter your patient’s data on your behalf, please email contact@cart-wheel.org with their name and email address. Your staff member will be given their own CART-Wheel.org account which they can use to enter your patient’s data. You will always be able to view the data your staff have entered.

What if I don’t have time to finish the questionnaire?

You do not have to answer all questions in one session. Return to the CART-Wheel.org website to login, view your patient’s questionnaire and enter further information at any time. You can continue the questionnaire or edit information already entered.

Can I get someone else to fill out the questionnaire on my behalf?

Yes, with your permission. If you would like any members of your treatment team to be able to enter your patient’s data on your behalf, please email contact@cart-wheel.org with their name and email address. Your staff member will be given their own CART-Wheel.org account which they can use to enter your patient’s data. You will be able to view all data your staff have entered.

Can I edit what I’ve entered in the questionnaire?

Yes. You can navigate backward and forward through the questionnaire as much as you like, adding new details or editing what you’ve already entered.

You can:

  • Use the 'Summary' Link in the top right corner to get an overview of which questions you have already answered and navigate back to any question for viewing or editing using the 'Edit' buttons.
  • You can also navigate between questions by clicking on the question number at the top of the page. Hover your cursor over the number to see the title of the section.

What if I’ve made a mistake?

If you make a mistake you can go back to the question and edit your answers.

The summary screen gives you a good overview of which questions you have already answered and also provides you with 'Edit' buttons that will take you back to the question for viewing or editing. Alternatively you can navigate between questions by clicking on the question number at the top of the page. Hover your cursor over the number to see the title of the section.

What if my patient has made a mistake? Can I correct any errors?

The CART-Wheel.org questionnaire was set up as a consumer-driven project. It’s important that information entered by your patient is not lost or over-written. For that reason, information entered by your patient is preserved as the 'Patient Record'. The 'Latest Record' contains a combination of your entries and your patient’s entries. Your patient is able to view any information you enter into the questionnaire and they can elect to save it as their 'Patient Record'.

Do I have to complete the pages in order?

No. You can navigate backward and forward through the questionnaire as much as you like.

You can:

  • Use the 'Summary' Link in the top right corner to get an overview of which questions you have already answered and navigate back to any question for viewing or editing using the 'Edit' buttons.
  • You can also navigate between questions by clicking on the question number at the top of the page. Hover your cursor over the number to see the title of the section.

Can I print out what I’ve done?

Yes. The summary page at the end of the questionnaire can be printed for your patient’s records or saved to your computer as a Word-file.

What if I am not sure of an answer?

If you don’t know the answer, please simply leave the field empty or select 'I don’t know' or 'Other' if that option is provided.

Must I answer everything? Can I leave things out and come back later to enter the info in?

No, you do not need to answer everything in the questionnaire. You can answer the questions you know and come back at a later time when you have more information. If you don’t know the answer, you can leave the field blank.

What if my email address changes?

If you wish to change your preferred email address, login to CART-Wheel.org and click on 'My Login details' at the top right of the page. You can also change your password at 'My Login details'.
