This information is for those who are filling out the questionnaire on behalf of the participant (the proxy), for example you may be a friend or relative of a patient who has a rare cancer.
The way in which the questionnaire has been structured is such that the questions are all directed at the participant ie "When were you diagnosed with your tumour?". If you are filling out the questionnaire on the participant’s behalf please interpret them as if you were the participant and answer accordingly.
If the participant is unable to give consent for their information to be stored and studied as part of this project and you are the legal next of kin then please read and fill out the appropriate consent form.
